Family Minor Waiver

This link from the SCA.org website provides a form that parents with more than one child may complete for all of their children (instead of completing separate forms for each child). One form must be used for each specific parent/legal guardian and their children -- it may not be used for a random assortment of children belonging to a random assortment of adults. Complete the form and present it at the troll booth for an event, or at any fighter practice at which the minors take part in combat-related activities.

Temporary Guardian Form (Arizona Residents)

This form is used to designate a temporary guardian for a minor attending an event, and is required if the parent/legal guardian is not attending the event with the minor. Two (2) copies of it must be signed by the parent/guardian and notarized before coming to the event, and presented at the Troll Booth when a site fee is paid for the minor.

Medical Authorization for Minors

This form is used to provide medical authorizations for a minor attending an event, and is required if the parent/legal guardian is not attending the event with the minor. Two (2) copies of it must be signed by the parent/guardian and notarized before coming to the event, and presented at the Troll Booth when a site fee is paid for the minor.
